The CAP Complaint Notification Form shall be used to document complaints with respect to utilities and industrial facilities, such as, but not limited to:
Discharges
Noise
Lighting
Odors
Poor Housekeeping
Please fill out the form as follows:
Document the date of the complaint and the date the incident took place.
Enter the your name, address, and phone number if you would like the result of our investigation. If you do not wish to reveal your name, check the "Anonymous" box.
Enter the utility or industrial facility and/or the location where the incident occurred. Be as specific as possible.
Enter a description of the event in the box provided giving as much detail as possible.
Indicate which Safety/Environmental issues are involved by checking the appropriate boxes.
